Registration

Registration for students new to Thompson Middle School takes place during the months of July and August. A registration packet is available in the Guidance Office. If you arrive during the school year, you must phone the Guidance Office for an appointment. To expedite the registration process you will need to bring (unofficial copies accepted):

editImmunizations and Health Records:
You must provide documentation that your child/children has/have received the following immunizations and or physical exam:
  • DTP/DTaP/DT/TdAP: 4 doses (additional Tetanus booster for grades 7/8 if not within the last 5 years)
  • Td (tetanus and diphtheria): Booster dose if it has been 5 years or more since the last dose of diphtheria-tetanus containing vaccine
  • Polio: 4 doses
  • MMR: 2 doses
  • Hepatitis B: 3 doses
  • Varicella (Chicken Pox): 1 dose or Physician's evidence of disease
  • Physical Exam: Grade 7 - required 12 months before to 6 months after school entry. All students in grades 6-8 entering from out-of-state are required to have a physical exam within one year.
editProof of Newport Residency: rent receipt, utility bill, RI Driver's License or RI DMV Identification Card, military housing letter.
editReport Card/Transfer Card from your child's/children's previous school.
