Registration Information
2007-2008 School Year Registration
WHO SHOULD REGISTER: All pupils entering Kindergarten through Grade Five who are not presently enrolled in the Newport Public Schools.
Children must be age five (5) on or before September 1, 2007.
THE FOLLOWING DOCUMENTATION IS NEEDED FOR REGISTRATION:
(Children CANNOT be registered without these items)
A. A Birth Certificate must be presented on the day of registration for each child entering the Newport School System.
PLEASE NOTE: A child must be five years old on or before September 1, 2007 for kindergarten registration. State law requires that a child must have completed a state recognized or accredited kindergarten program to be eligible for enrollment in first grade in September of 2007.
B. IMMUNIZATIONS AND HEALTH REQUIREMENTS:
- DPT: 4-5 DOSES [4 if 4th dose given after age 4]
- POLIO: 4 DOSES
- MMR: KINDERGARTEN – GRADE 5: 2 DOSES
- LEAD SCREENING: KINDERGARTEN ONLY
- VISION SCREENING
- HEPATITIS B: KINDERGARTEN – GRADE 12: 3 DOSES
- VARICELLA (CHICKEN POX): KINDERGARTEN – GRADE 12: 1 DOSE OR PHYSICIAN’S EVIDENCE OF DISEASE
- PHYSICAL EXAM: REQUIRED 12 MONTHS BEFORE TO 6 MONTHS AFTER SCHOOL ENTRY
State law does not allow students to attend school who do not have their required immunizations.
C. Proof of Residency (rent receipt, utility bill, letter from military housing office, etc.).
D. Report Card/Transfer card from previous school.
Registration Packets available at the Administration Building after March 13, 2006.
