To expedite the registration
process, the following documents are
required (unofficial copies
accepted if in a sealed envelope):
• Immunization/shot record – required by RI law: 5 doses of DPT; 4 doses of OPV; 2 doses of MMR
Grades 9 & 10 - 3 HEP B shots & proof of having had Chicken Pox Disease or Varciella Vaccine
• A copy of a recent physical exam is required - (12 months before to 6 months after school entry)
• Photocopy of cumulative record (If not presented scheduling will be delayed)
• Teacher letters of recommendations for advance placement courses or special courses
• Information of grading system from previous high schools attended:
1. letter grade breakdown
2. numerical equivalent
• Course outline book from previous school/s (for course comparison)
Proof of immunizations must be provided before registration can be complete. Students will not be allowed to attend Rogers until this documentation has been provided.
Every entering or transfer student must present documentation of a physical exam by a medical examiner to Health Services. Accepted are new or recent physical exams, including seventh grade physicals, and sports physicals that are less than one year old.
(Newport Community Health Center, 19 Broadway, provides services with payments adjusted on a sliding scale based on income.)
